School Trip Application
Requirements

Form must be in triplicate
o Name and contact
information for the primary trip organizer
o Adequate First Aid
kit
o In case of water
trips
o Name of certified
lifeguard; or
o Tour operator
o A written justification
letter

Supporting Documents
o List of participating
students, teachers and parents/guardians is attached to the form
o A complete itinerary for
each day is attached to the form
o Correspondence (permission,
approval letter, invitation) to the school from the place of visit is attached
to the form
o Appropriate student
activity sheet to be completed during or after the trip
o At least on signed
consent for from a parent/guardian is attached
o A signed commitment
from the school to report any unforeseen incident(s) and or deviation from itinerary
before or during or after the is evident
o A chaperone
chart/plan for overnight and or water trip is attached (if applicable)
o All trips away from
the school compound should have a written parental consent and notification to the
District Education Center
o Weekend trip should
be approved only if its purpose is justifiably linked to the objective of the
trip
o An email notification
is to be submitted to the Deputy Chief Education Officer with responsibility for
District Education Centers for any approved water, overnight or out of Country trips.
o An email notification
is to be sent one Center Manage to another for cross District trips.
o A disclaimer on trips
is to be seriously considered before approvals can be awarded,
