Requirements
The following must be submitted with the application form (NEW APPLICANT)
- Facility floor plan for the business
- Business certificate of registration
- Proof of citizenship of applicant/s and pharmacist/s to be employed (Notarized birth certificate or passport)
- Photo Identification of applicant/s and pharmacist/s to be employed (Notarized Social Security or Drivers Permit)
- Notarized and authenticated original degree, diploma or certificate for all pharmacist/s to be employed. The final signature of authentication must be that of the Embassy of Belize or British High Commission in tat country.
- Notarized and authenticated copy of Chemist Druggist Certificate (for each pharmacist employed)
- Official Translation of all documents to English Language, if documents are in any other language
- Letter of employment verification between owner and Pharmacist/s (if owner of the pharmacy is not the pharmacist)
- Proof that applicant is able to read and write English (for those from non English speaking country)
- Police Record (not older than 6 months) for all workers and owner from the Belize Police Department
- For non-Belizeans a Police Record from country of citizenship and from Belize, if they living in the country for less than six (6) months
- List of medication in stock (retail pharmacy only) table which contain: Brand name, Description, active ingredients (generic name), strength/concentration, intended for use in adults or children, name and place of manufacture.
The following must be submitted with the application form (RENEWAL)
- Copy of most recent approved license from the Department of Health Service, Ministry of Health
- For the applicant and pharmacist, their most recent police record from the Belize Police Department (not older than six (6) months)
- If any new pharmacist/s will be recruited at the time of renewal, then the first 2 steps above will be applicable
- Receipt of payment for registration of pharmaceutical establishment.
Application process will take 4 - 6 weeks after submission.
