Requirement

Adult (16 years and over) New and Renewal

  • A copy of all documents along with the original documents must be presented.
  • Complete application form
  • Form 4A
  • Form 4B

Form 4A

Must be endorsed by one of the below:

  • Belize Justice of the Peace
  • Medical Practitioner
  • Notary Public
  • Attorney at Law

Form 4B

Must be endorsed by one of the below:

  • Member of the House of Representative
  • Chief Executive Officer of any Department of the Government
  • Head of Department of any Department of the Government
  • Licences Teacher 

The applicant for a child under the age of 16 years old is either:

  • The parent or
  • The legal guardian

Application need to sign by applicant (Parent/Guardian) on behalf of the child who is under the age of 12
A child 12 years or older must sign the application form as well as the applicant (Parent/Guardian)

Supporting Documents

  • Born Belizean - Original Birth Certificate
  • Naturalized Belizean - Original Nationality Certificate
  • Adopted - Original Adoption Certificate
  • Name change - Original Deed Poll (Registered in Belize)
  • Photo Identification of Applicant (Parent/Legal Guardian) stating name exactly as it appears on Child's Birth Certificate, Child's Nationality Certificate, Child's Adoption 
  • Photo Identification for Applicant stating name exactly as it appears Valid Belize Social Security Card, Voters ID or Passport
  • 2 Passport size photos, white background 2 X 2 certified and recommended with the quote "I certify that this is a true likeness of the applicant" signed and stamped

If previous passport does not reflect your legal status surname the following is required

Damage

  • All supporting documents as required for renewal of passport
  • Form 6
  • Police Report
  • Declaration form

Lost/Stolen/Mutilated

  • All supporting documents as required for renewal of passport
  • Police Report
  • Form 6
  • Declaration form of lost, stolen or mutilated Belize Passport
  • In case of an emergency a temporary passport can be facilitated. Persons would need to complete a separate application form to access this service at a cost of $30 Bz

Cost of Minor

Regular Fees

  • $30.00 - four (4) weeks or twenty (20) working days

Damage

  • $55.00 - four (4) weeks or twenty (20) working days

Mutilated

  • $55.00 - six (6) weeks or thirty (30) working days

Expedited Fees

  • $230.00 - twenty four (24) hours (Return next day after application submission)
  • $130.00 - Forty Eight (48) hours (Return within 3 days after application submission)
  • $80.00 - Five (5) days (Return within 5 days after application submission)
  • Medical emergency fee is $80.00 and is process within five (5) days.

Note that this requires official and original doctor medical certificate
